Is there any reason that TVK NHMSYS0000551128 is deemed a badly formed name?

It looks fine to me.

Is it possible to make TVK NHMSYS0000551128 a correctly formed name or is this also fixed and irreversable?

If not, is it possible to make TVK NBNSYS00001711464 which is flagged as correctly formed the synonym of NBNSYS0000005891? I have a feeling this is where TVK NBNSYS00001711464 should be in the hierarchy? - I could be wrong though!

I raise this as via Web Services it shows up as a problem on our web site. See URL http://www.mothscount.org/maps/94/moth_distribution_maps.html?filter=P - if this was correctly formed it would be prefixed with "Twin-spot Carpet" and be found under the filter "T" alongside its subspecies.

I hope this makes sense.

Once again, well spotted. NHMSYS0000551128 should be marked as Well-formed. The reason why 'Twin-spot carpet has 2 TVKS is that we (perversely perhaps) assign ranks to vernacular names so NHMSYS0000551128 is for the species and NBNSYS0000171464 is for the subspecies.
