I have found the following bug, which (in my copy) appears to be repeatable:
using the enter one species record form, if you include a comment, this does not appear in the finished record

This is a bug that has been around for some time. It is logged as Mantis 53. The comment is stored but sometimes it isn’t visible initially. When it isn’t visible, closing the data entry window (Enter a species record) enables the comment to be viewed, although if the observation is currently selected it will be necessary to select something else then return to the observation to see the comment. This is a nuisance but as the data is stored correctly it isn’t easy to give it a high enough priority to get it fixed. In my tests this problem didn’t occur if the Observation hierarchy was open when new observation was entered. Data Entry – Enter a species record used to put the comment on the sample and taxon occurrence but it no longer puts it on the sample so that has been rectified.
